Our staff has extensive clinical experience providing care to children, adults, seniors and newborn babies. On admission, our registered nurse and/or therapist will conduct a full and comprehensive assessment to determine your current medical condition, history and your wishes. Our nurses will work in conjunction with your physician to develop a plan of care specific to your needs. Our treatment and clinical services include:

Wound Care

Forming a partnership with the patient, caregiver, personal physician, physical therapist, WOCN/ET certified nurses, and home health aides enables us to address every aspect of wound care.

Infusion Therapy

Our team works closely with infusion companies to deliver treatment in the comfort of the patient’s home. We will communicate care information to the infusion company and your healthcare providers to ensure that you or your loved one is receiving the most effective plan of care.

Infusion therapy

Chronic Care

Living with chronic disease can be challenging, but patients who take an active role in their care can make more informed choices and are less likely to be re-hospitalized. Our Chronic Care Management programs focus on involving patients in the healthcare process so they can achieve more independence and a better quality of life.


The goal of each of our programs is to decrease hospitalizations in the long term. We use the most current medical evidence to develop our clinical and patient education, and plans for patient care.


Chronic Heart Disease

All heart conditions, no matter how complex, are managed under our Heart Disease Home program. Revolutionary Home Health & Hospice clinicians provide detailed assessments and education to help patients successfully manage medication, symptoms, mobility, and other aspects of care. If needed, they will coordinate other services, including physical therapy, special equipment, and nutritional education. Our goal is to provide the information and skills needed to stay safe in the home after home health care has ended.


Diabetes

Patients with Type 1 and Type II diabetes have different needs for education and medical supervision. Revolutionary Home Health & Hospice's skilled clinicians are prepared to provide customized support for each of these conditions based on the individual patient’s needs.


The education Revolutionary Home Health & Hospice provides is valuable whether the diabetes is newly diagnosed or longstanding, as information for the successful management of diabetes has expanded greatly in recent years and treatment is better than ever before. The Revolutionary Home Health & Hospice clinician focuses on each patient’s specific needs, improving quality of life and enabling patients to achieve their best health status. Our team includes diabetic educators.


COPD

Living with Chronic Obstructive Pulmonary Disease (COPD) is stressful for patients and their families. Our skilled clinicians provide care, therapies, and education that improve the patient's ability to manage COPD and enjoy an improved quality of life.


Patients and caregivers are encouraged to actively participate in creating the management plan and are trained in the skills needed to comply with the plan. Adhering to a structured care plan will help prevent or delay complications and achieve the best possible outcomes while reducing hospitalizations.


Cancer

Patients and families coping with cancer have the support of Revolutionary Home Health & Hospice clinicians who provide ongoing assessment, timely treatment, and coordination with physicians, oncology clinics, and other healthcare providers.


Our skilled clinicians provide information and education to support the patient and family, improve the patient’s quality of life and decrease the overall stress level that this condition creates. With reliable information about the disease and its management, patients with cancer can control their condition and minimize its impact. Our specialized infusion nurses can assist with receiving chemotherapy agents at home. We provide these services to all age groups.


Pain Management at Home

Chronic pain significantly impacts the quality of life for patients and their families. Our pain management program is designed as a team approach, actively involving patients, caregivers, skilled nursing professionals, and other therapeutic disciplines as needed. Together, we develop a holistic treatment plan based on each patient’s condition.


Using the best clinical evidence currently available, Revolutionary Home Health & Hospice clinicians work with the patient and family, teaching the skills needed to help them continue to control pain after the home health care has ended.

Chronic care

Therapy

Revolutionary Home Health & Hospice physical and occupational therapists will develop home exercise programs aimed at improving the patient's musculoskeletal system. We will also educate the patient, family, and caregiver on safety and mobility. If necessary, our therapist will work with durable medical equipment companies to assess the need for assistive devices.


Our speech therapists are trained to work with your physician to develop a plan of care to improve overall speech and communication and enhance the patient’s ability to form sounds and use the right words. We also specialize in working with stroke patients and other conditions that affect swallowing.


Post Operative Orthopedic Care

Our therapists are highly skilled and trained to assist you in your recovery after any orthopedic procedure. We can assist any age on the road to recovery. Skilled in the care of joint replacements, orthopedic injuries, and complex orthopedic disease patients, our clinicians provide skilled nursing and licensed therapeutic care to support patient recovery based on established clinical protocols. Our goal is to assist the patient in reaching the highest possible level of independence in the comfort of their home.


Fall Prevention

Falls are the leading cause of nonfatal, unintentional injuries treated in a hospital ER department–and the leading reason for admission to a skilled nursing facility. Revolutionary Home Health & Hospice has a comprehensive geriatric program that evaluates each patient to develop a systematic approach to treatment. The goal is restoration and management of the balance disorder to facilitate safe, independent living within the home.


Chronic Issues

Our clinicians coordinate services for the management of acute and chronic diseases including congestive heart failure, chronic obstructive pulmonary disease, diabetes, and cerebral vascular accident. Our clinical team uses therapeutic skills to assist the patient in improving strength, mobility, balance, swallowing, and communication disorders.

Therapy

Maternal Child Health

Under this program, registered nurses provide care and education to expectant mothers during pregnancy and new mothers after delivery. Specifically, the nurses assess and develop plans of care for:


  • Teaching prenatal care, postnatal care, newborn care, and CPR.
  • Gestational hypertension or pregnancy-induced hypertension (PIH) – A condition that affects women after 20 weeks gestation without the presence of protein in the urine.
  • Gestational Diabetes (GDM) – A condition in which women without previously diagnosed diabetes exhibit high blood glucose levels during pregnancy (especially during their third trimester).
  • Hyperemesis Gravidarum: This is a complication of pregnancy characterized by intractable nausea, vomiting, and dehydration often leading to malnutrition and other serious complications, such as fluid or electrolyte imbalances.
  • Preterm birth.
  • Mom and baby assessment.
  • Teaching, support, and referral to community services.
Maternal health

Pediatric Care

Our Pediatric Care Program consists of experienced nurses who have the sensitivity and the understanding of working with young ones. Services under this program can either be intermittent care or they can be long-term pediatric home care. Long-term care is provided under our home care services. Services under this program include:


  • IV therapy
  • Chemotherapy injections at home
  • Diabetic teaching
  • Tracheostomy care
  • Ventilator support and monitoring
  • Feeding tube management
  • In-home asthma management
  • Wound care
  • 1:1 Nursing to school (provided under home care services)
Pediatric care

Behavioral Health

Patients with psychiatric conditions have special needs. Frequently, psychiatric illness exists in tandem with physical illness, requiring nursing care that is specific to both conditions. Revolutionary Home Health & Hospice's psychiatric nurses are trained to provide care for the total patient, including chronic disease conditions and education for the patient and caregiver.


Working with the psychiatrist and/or primary care physician, our clinicians make sure the needs of the patient are being managed effectively.


Our Pediatric program also provides services to younger psychiatric patients who are placed on disability due to the severity of their mental illness. Consistent home care, provided by our psychiatric nurses, reduces inpatient readmissions and teaches the patient and caregiver to better manage the illness in the home.

Nursing

Medical Social Workers

Medical social workers can assist you or your loved one in dealing with emotional concerns resulting from illness. Our skilled social services include:


Identify community resources including county, state and government funding opportunities. This may include home-delivered meals and Meals on Wheels.


  • Assist and counsel the patient to prepare for long-term care
  • Provide emotional support to the patient
  • Assist the patient to purchase or rent medical equipment
  • Help strengthen the relationship between the patient and his/her family
  • Explain Medical bills and the billing process


Our social workers also organize small social support teams, consisting of patients who share similar challenges, allowing them to share information and overcome these challenges.

Medical worker

Psychiatric Care

  • Frequently, psychiatric illness exists in tandem with physical illness, requiring nursing care that is specific to both conditions. That’s when your physician may add a psychiatric nurse to the team. 
  • As part of our Psychiatric program, a psychiatric nurse can help patients and their caregivers cope with dementia, Alzheimer's, schizophrenia and a variety of psychiatric illnesses.
  • Frequently, these specialized nurses work with those suffering from anxiety, grief and depression – problems that often come about as a result of a stroke or a chronic condition. Home psychiatric nursing is especially important when these conditions lead to an aversion to leaving home.
  • Senior mental health, particularly depression in the elderly, is a growing concern in the medical community. Research has shown that depression is twice as common in the home health population as in the primary care population. The psychiatric nurse works with the healthcare team to treat depression so that the patient has the energy to deal with their other medical conditions.
Elderly care

Home Health Aides

Revolutionary Home Health & Hospice home health aides work under the direct supervision of a licensed nurse or therapist. They provide intermittent care to patients that are receiving skilled nursing care or some other form of therapy. Home health aide assistance includes:


  • Bathing and dressing the patient
  • Provide skin, hair and mouth care
  • Assist the patient in walking and exercising
  • Assist the patient in preparing meals
  • Incontinence and catheter care
  • Laundry and cleaning to maintain a safe environment for the patient
  • Emotional and social support
  • Pet care
  • Escorting to appointments
  • Observing and checking the patient’s respiration, temperature and pulse rate


The assistance provided by the home health aide is reviewed by a licensed nurse or therapist regularly. The licensed nurse or therapist in turn develops and updates the patient’s plan of care in consultation with the patient’s physician.
